Members registration

Mathiwos Wondu Foundation (MWF) is a local non-profit organization established by Mr. Wondu and 15 founding members in memory of Mathiwos, who passed away from leukemia at the age of 4. For over 21 years, MWF has worked to advance cancer and non-communicable disease (NCD) prevention, screening, early diagnosis, treatment, and psychosocial support, reaching more than 5,000 individuals and families through its initiatives.

This membership form enables individuals and organizations to formally join the MWF membership network and contribute to our mission.

Membership Categories

• Regular Membership
  • Tier I – 1,000 ETB / year
  • Tier II – 3,000 ETB / year
  • Tier III – 5,000 ETB / year
• Life Membership: 20,000 – 30,000 ETB
• Honorable Membership: ≥ 30,000 ETB

Registration Process

1. Fill out your basic information.
2. Pay your chosen contribution via CBE Account: 1000007229686 (Mathiwos Wondu – YeEthiopia Cancer Society).
3. Attach the payment receipt and upload a passport-sized photo using this form.
4. Upon completion, you will receive an electronic membership certificate via email or Telegram shortly.
